At the UCLA Center for IBD, it is our mission to completely control your disease activity, offer safe therapeutics and design our care programs specifically to your situation. To achieve this mission, we adhere to the principles of Value-Based Health Care, aiming for an annual improvement of disease control, quality of life, and daily productivity. We measure the value of your care with a Value Quotient or VQ which captures the value of healthcare services to individual patients over time and correlates this with its associated cost. The public portion of the UCLAeIBD application invites you to learn about our unique value-based care program; the first in the country to focus primarily on adding value to your life, despite your chronic condition. You will also learn about our exciting research programs, program patient testimonials, and get to know the UCLA IBD team! You will also be able to follow us directly on Twitter and Like us on Facebook. The signed in portion of the UCLAeIBD application is designed specifically for enrolled patients at the UCLA Center for IBD. As a program patient, you will be able to perform a variety of tasks - from messaging your healthcare team to checking when your labs are due - you ll be able track important aspects of your IBD management! You will also be able to complete your home care at any time so we can closely monitor your symptoms and VQ. As a program patient, UCLAeIBD allows you to: Check your current and historical VQ Contact your healthcare team View your individualized Care Program: Tight Control Scenario Complete your home care View upcoming labs, clinic visits, scheduled home care, and procedures Review and confirm IBD medication Application functions are interactive, allowing you to place a call for an appointment, get directions, and view entries over time.
